Our Services

We liaise with families who may be hundreds of miles away, answering questions and addressing the complex questions that they have about their loved one’s needs.

Data shows that 77% of adults 50 and older want to remain in their homes for the long term — a number that has been consistent for more than a decade. However, organizing your loved one’s care needs to help them age in place often comes with a cost: stress, overwhelm, confusion, and even guilt. The important questions to ask yourself as your love ones age are:

Do you find yourself consumed by the ever-evolving physical, medical, and emotional needs of your aging loved one?

Are you constantly feeling guilty for feeling like you are not doing enough to ensure their comfort
while dealing with burnout from juggling the rest of your daily responsibilities?

Does the topic of how do I keep my loved one safely at home feel too overwhelming to even think about?

We provide a variety of geriatric care management services, including:

Care Management is Personal

A human connection is at the heart of what we do.
With in-person or virtual assessments, we thoroughly understand each of our seniors’ needs.

Our assessments allow us to create short- and long-term care plans, including:

  • Selecting the appropriate level of housing.
  • Performing appropriate crisis intervention services.
  • Addressing care concerns and differences of opinion about long-term care planning.
  • Assisting families adjust, cope, and problem-solve around long-distance and in-home caregiving.
  • Offering expert knowledge of the local resources in their communities and how these services are accessed.
  • Strong and effective advocacy for clients and families, promoting their wishes with healthcare and other providers.

A care plan tailored for each individual’s circumstances is prepared after a comprehensive assessment.
The plan may be modified, in consultation with client and family, as circumstances change.

My mom mentioned that she has some questions about a physician appointment and an insurance issue, and when she mentioned these you took action without being asked, looking into things and explaining to her in a way that she understood. She said that your help was “a godsend” and “what she knew was amazing!”

My mom and I are truly appreciative of the numerous extra steps and efforts that you have displayed towards us. You are an asset and you make a difference in the lives of the families that you assist.

Kiersten was wonderful! She understood everything we told her with great compassion. Our situation is a bit unusual but knowing we can call on her again if needed is comforting. The sense of not being alone in this situation makes it easier to handle.
