Meet Kiersten Dortone, a dedicated and compassionate geriatric care manager who brings a wealth of expertise and a decade of experience in the field of geriatrics. With a strong background as an occupational therapist, Kiersten has made it her mission to enhance the lives of older adults and their families through personalized care and comprehensive support.

Kiersten’s journey in geriatric care began with her passion for improving the lives of older adults and recognizing the unique challenges they face. After completing her Master’s degree in Occupational Therapy at Temple University, she embarked on a fulfilling career that has allowed her to make a positive impact on the lives of countless individuals.

As an occupational therapist, Kiersten has a keen eye for identifying and addressing the practical aspects of daily living. She excels in assessing an individual’s functional abilities, identifying areas of improvement, and developing personalized strategies to promote independence and safety. Her expertise extends to assessing home environments, recommending modifications, and connecting individuals with appropriate assistive devices and technologies.

Kiersten’s commitment to personalized care and attention to detail sets her apart as a geriatric care manager. She takes the time to listen and understand the unique needs, preferences, and goals of her clients and their families. Our assessments allow us to create short- and long-term care plans, including:

  • Performing appropriate crisis intervention services.
  • Addressing care concerns and differences of opinion about long-term care planning.
  • Assisting families adjust, cope, and problem-solve around long-distance and in-home caregiving.
  • Offering expert knowledge of the local resources in their communities and how these services are accessed.
  • Strong and effective advocacy for clients and families, promoting their wishes with healthcare and other providers.

A care plan tailored for each individual’s circumstances is prepared after a comprehensive assessment.
The plan may be modified, in consultation with client and family, as circumstances change.
